By typing my name below I also certify that my child is healthy and able to participate in athletic activities. I am aware that there are significant risks and hazards involved in all aspects of athletic training, which include injury, sickness (including COVID 19), and death due to the negligence on the part of my child, other participants, Nampa School District, Mettle Sports Training, LLC, Mettle Building LLC, NNU, Paul Rush Basketball LLC or Idaho Stags (“Participants”). I hereby assume full responsibility for any injury or death that may result from my child’s participation in Idaho Stags. I hereby release Nampa School District, Mettle Sports Training, LLC, Mettle Building LLC, NNU, Paul Rush Basketball LLC or Idaho Stags from any and all liability, claims, demands, actions or rights of action which are related to, arise out of, or are in any way connected with my child’s participation in Idaho Stags (“Activities), including those allegedly attributed to the negligent acts or omissions of Participants. This waiver shall be binding upon my and my child’s successors, representatives, heirs, executors, assignees, or transferees. I accept full financial responsibility for any injury that my child may cause to Idaho Stags or any Participant due to my or my child’s negligence or malfeasance. Should Idaho Stags or a Participant, or anyone acting on their behalf, be required to incur attorney’s fees and costs to enforce this waiver, I agree to reimburse them for such fees and costs. I further agree to indemnify and hold harmless the Participants from liability for the injury or death of any person(s) and damage that may result from my or my child’s negligent or intentional act or omission while participating in activities.
